We know you have many choices when choosing an Otolaryngologist in Southfield, MI, so we have made requesting an appointment a simple process via our Web site. If, for any reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. Charges may be incurred for appointments cancelled less than 24 hours before scheduled appointment time.
Our office accepts a variety of HMOs, PPOs, and other health plans. Please call our office to verify acceptance of your insurance carrier.
It is the responsibility of the patient or responsible party to see that all charges are paid in full. We participate with most insurance programs, but it is the patient’s responsibility to know what their policy provides. As a courtesy to you, we will file all medical claims with the primary and secondary insurance. However, you must provide us with current copies of your insurance and notify us immediately if there are changes in this information. It is your responsibility to obtain any referrals required by your insurance company and update them as needed. If you do not have a current referral you may be asked to reschedule your appointment or sign a waiver stating that you will be responsible for payment of charges. There is a fee when we send statements.
Deductibles must be paid. It is a legal contract between you and you insurance company. We are required to collect your deductible. If a deductible is identified, it is to be paid at the time of the visit.
(HMO, PPO) are to be paid at time of service. If you are unable to pay at the time of the visit there may be a billing fee.
We submit and accept assignment on all Medicare claims. As a courtesy, we will file to your secondary insurance. You are responsible for co-payments and deductibles.
May be made by Visa, MasterCard, check or cash. Many buildings have an ATM in the lobby.
If you do not have health insurance, a substantial payment of will be collected at the time of your first visit. Payment plans are discouraged, but can be made in situations of special need. A substantial payment will be collected at time of each return office visit. Any outstanding balance is due immediately upon receipt of statement. There is a fee for sending statements
Caring for children is one of our highest priorities. In divorce situations, the parent who brought the child into the office is responsible for payment of the bill. We will submit to the necessary insurance carriers.
An individual’s financial situation is respected in our office. If there are financial difficulties we will try assist you, if possible. Accounts that are 45-90 days past due may be referred to a collection agency, without notification, unless payment arrangements have been made with our business office. If an account is referred to a collection service there will be a substantial fee added to the balance. If you have a financial hardship, please let us know so that we might set up payment arrangements. Finally, your insurance policy is a contract between you and your insurance company. You must know your plan’s coverage, restrictions and requirements. It is your responsibility to communicate with the insurance company if you are not happy with your insurance company's determination of benefits for your claim. You may be responsible for paying denied claims and charges.
